Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team. Knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Knowledge of psychosocial treatments in order to provide education to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals Ability to setup and/or initiate therapeutic services to individuals, groups and families. Ability to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements. Participate in evaluate of his/her own practice through participation in professional peer review case conferences, research studies, or other organized means. Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA. Provides telephone triage and referrals as needed. Facilitates coordination of care with various providers and programs within VA and community. Provides resources and referrals to Veterans and/or family member. Provides resources/referrals to VA colleagues and community partners as needed . Facilitates patient and family education groups as needed. Provides recovery oriented interventions, with focus on individual Veteran and or family needs. Completes at risk documentation for Veteran when needed or necessary. Provides assessments, referrals and coordinates transfer to emergency placement/housing when needed. Provides on-site social work services to the greater healthcare system, based on need. May assist in development and execution of ongoing program evaluation and data driven quality improvement projects to monitor the effectiveness of the clinical program as needed or indicated. Complies with all electronic documentation procedures, including timeliness standards for documentation and proper patient check-out procedures (e.g. entering diagnostic and procedure codes). Complies with national and local performance measures including timely healthcare screening and completion of clinical reminders. Coordinates screening of Veteran's to determine level of acuity. Coordinates screening of Veterans for mental health and other VA specific programs and documentation per VA guidelines and facility policy. Providing education on mental illness for Veterans and their families. Developing treatment plan with the Veteran based on assessment. Functions as an advocate when appropriate and feasible while encouraging the Veteran and family to advocate on their own behalf thus fostering a sense of independence and empowerment.
